A Game-Changer in Pediatric Medicine: Local Kindergartener Discovers the Joy of Hearing After Groundbreaking Ear Surgery
Imagine the frustration of living five years of your life hearing the world muffled, as if you were submerged underwater while wearing headphones. You might feel vibrations, but when it comes to understanding conversations, everything blends into an indecipherable haze. This was the daily experience of five-year-old Eliza Fowler until technology came to her aid, allowing her to step foot into kindergarten with a newfound ability to hear clearly.
In a remarkable development that underscores the advancements in pediatric healthcare, Eliza was born with a rare condition known as bilateral microtia. More commonly referred to as "small ear syndrome," microtia involves the underdevelopment of the outer ear and ear canal. As Eliza's mother, Jennifer Fowler, describes it, "She doesn’t have a properly formed ear canal, and her ear resembles a butterfly wing."
Microtia can manifest in four different levels of severity, ranging from a slightly small ear to the complete absence of an external ear. According to Cedars-Sinai—a prestigious nonprofit hospital and research institution in Los Angeles—the condition may involve either a partially formed ear drum and canal or none at all.

Eliza specifically has what is classified as grade three microtia, often described as severe microtia, where her ear is small and shaped like a peanut. While patients typically retain a partial earlobe, there is no external ear canal or ear drum, making this the most prevalent type among cases of microtia.
At just nine days old, Eliza underwent her first surgical procedure, paving the way for what would become a series of surgeries and consultations. Her family was soon introduced to Lewin Ear Reconstruction, a specialized practice in California focusing on microtia. During their initial meetings—with the help of FaceTime— Dr. Sheryl Lewin, a plastic surgeon skilled in the pioneering Porus Implant Ear Reconstruction (PIER) technique developed by her, provided insights into how to achieve the best outcomes with minimal scarring while creating ears that look natural.
Typically, most surgeons delay reconstructive surgery until the ear has fully developed, which occurs around the ages of five or six. Thus, for the earlier years of her life, Eliza utilized a bone-anchored hearing aid (BAHA) secured under a headband that became an everyday accessory for her.
Before the much-anticipated surgery, Eliza and her mother made multiple trips to California to consult with Dr. Lewin. In June, after reconstructive surgery to form her ear, Eliza received an Osia cochlear implant. This innovative device, placed beneath the skin and directly on her skull, transmits sound vibrations through the bone, conveniently bypassing the middle ear for enhanced hearing capabilities. The operation, which lasted a grueling ten hours, was a significant milestone in her life.
The emotional moment Eliza first perceived her mother's voice distinctly was nothing short of magical, bringing tears to her eyes. The celebration that followed, with her family and surgical team singing “Happy Birthday” to her new ears, encapsulated the profound relief and joy they all felt. Although sometimes she misses her previous headband, it's clear that Eliza is thrilled with her new, larger ear.
As Jennifer Fowler shared, "She absolutely loves her beautiful new ear and admires it frequently. It’s just charming to see her check on it every day."
Eliza has already begun to appreciate the rich tapestry of sounds around her, excitedly exclaiming "S, ssss, snake" to her mother. However, as Jennifer pointed out, Eliza is still adjusting to hearing higher-frequency sounds more clearly.
One of the standout moments post-surgery occurred during bath time: as Eliza splashed in the tub, her delight was palpable, lighting up her face as she exclaimed, "Woah, I really like this!" Her mother recalled the sentiment, explaining, "This was a new experience for her because her BAHAs couldn’t get wet."
Eliza even joyfully shared her newfound abilities, saying, "Yeah, and I can play with my Barbies in the bathtub," as she relished the enchanting sounds her mermaid dolls made while gliding through the water.
Her remarkable transition continues with the joy of hearing the barks of her two dogs, Luna and Lincoln, more clearly than ever after receiving her implant. Despite her medical journey keeping her hobbies on hold, Eliza maintains her love for horseback riding, indulging in ice cream and enjoying her iPad. Notably, her implant conveniently syncs with her iPad so that she no longer needs to wear additional headphones, much to her delight. Eliza is enthusiastically looking forward to returning to her beloved horseback riding in the near future.
Amidst this journey, Eliza is supported by her two siblings who have traveled with her during her medical appointments and surgeries.
Jennifer Fowler, who was juggling night shifts as a remote OBGYN triage nurse until recently, mentioned the challenges of coordinating her work schedule, her husband’s obligations, and family appointments. She expressed deep appreciation for finally being able to focus more on her family.
Eliza's second and final surgery is scheduled for October 16, after which the family plans to spend 11 days in California for recovery. Jennifer attributes her ability to navigate Eliza’s diagnosis to the strong community connections and relationships she has built along the way.
When asked about her future aspirations, Eliza expressed a desire to become a doctor caring for children with broken bones or perhaps a veterinarian, highlighting her nurturing spirit.
